※Please prepare one copy of each of the following materials and mail them to the following address before December 10(Thu)(inclusive), 2020:

Institute of Applied Arts, National Chiao Tung University.

1. Grade Notice <photocopy>.
2. Transcript for all semesters in college <original>. (Please indicate your candidate number, application number in the upper right corner)
3. Graduation certificate <photocopy>; if you have not obtained a degree certificate, you must submit the <Affidavit Letter>.
4. Those who apply for enrollment in February 2021 must submit the “Application for Enrollment in February”.
